Rquesting Your information

You have a right under the Data Protection Act 2018 to be told whether the council holds any information about you and to receive a copy of that information, subject to exemptions. This type of request is known as a ‘Subject Access Request’. Please complete this form if you wish to access the personal information about you which is held by Luton Council.

This Form

This form asks you to provide your contact details, the information you are requesting and to enclose proof of identity. Before we can process your request, we must establish your identity. This helps to ensure that the council never releases information to anyone who should not have access to it.

To help us to establish your identity, your application must be accompanied by two documents proving your identity that you must upload copies of. If you do not have access to a scan or photograph of your ID now you should not begin this form. If you cannot upload a copy of your ID you should contact us using the details below to discuss how best to provide it:

official Identification showing a photograph of yourself eg a current valid passport or driving licence. If this is not available, your original or certified copy birth certificate
a utility bill or other official correspondence dated within the last three months showing your current address
